Additional forms for Coaches and Locker Room Monitors
The new VSAHA on-line background check for coaches and locker room attendants is now available. All Vermont coaches are required to complete this form as soon as possible. Also, in accordance with USA Hockeys new requirement for a locker room attendant, any team parent that will be acting as a locker room attendant is also required to complete the form.
